Comprehensive head-to-toe exams for all ages. We review your health history, order preventive screenings, update immunizations, and set goals for the year ahead. Your annual physical is the foundation of staying healthy — not just catching problems.

When you're not feeling well, you need to be seen quickly. We offer same-day appointments whenever possible for illnesses, infections, injuries, and other acute concerns. No waiting a week when you feel awful today.

A family nurse practitioner with over 25 years of healthcare experience, Jenny founded Burds Family Practice on one belief: patients deserve more time, more attention, and more genuine care than the traditional system allows. She earned her degree from Clarke University and has deep roots in the Peosta community.

Channah Chase
DNP · Nurse Practitioner

A Doctor of Nursing Practice graduate from Clarke University with 8+ years of nursing experience, Channah brings advanced clinical knowledge and a warm, patient-centered approach to every visit. She's a passionate advocate for the communities we serve in western Dubuque County.

Are you accepting new patients?

Yes — we are actively welcoming new patients of all ages. Call us at (563) 588-6659 or book online at burdsfamilypractice.com to get started. We'd love to be your family's healthcare home.

What insurance do you accept?

We accept most major commercial insurance plans. We also offer a Direct Primary Care membership for patients without insurance or those who prefer an alternative to the traditional model. Call us to confirm your specific plan.

Can you be my child's doctor AND my doctor?

Absolutely. We provide primary care for patients of all ages — from newborns through seniors. Many of our families have multiple generations under our care, which gives us a rich picture of your family's health history.

Do you offer telehealth appointments?

We offer telehealth for select visit types when clinically appropriate. Call our office to find out if your visit qualifies for a virtual appointment.

What is the difference between a regular patient and a DPC member?

Standard patients visit us under traditional insurance billing. Direct Primary Care (DPC) members pay a flat monthly fee and receive unlimited visits with no copays, same-day access, and direct contact with their provider. Ask us if DPC might be a better fit for your needs.

Do you offer sports physicals and school forms?

Yes — we complete sports physicals, school physical forms, camp physicals, and other required paperwork for children and teens. Call ahead to schedule and let us know what you need so we can have the right forms ready.
